Why is it that Dr. McCoy and Mr. Spock always get on each other's nerves--a lot?

They represent opposite sides of the pendulum. Spock is logic, while McCoy is pion. Kirk is the pendulum that swings from side to side as he makes his universe-saving decisions. Naturally, Spock and Bones would get on opposing natures would get on each others nerves.
